4 Population Health Trends Regarding EHR Adoption

The healthcare industry has been slower to leverage the benefits of electronic storage, but recent trends reveal US healthcare organizations are increasing electronic health record (EHR) implementation. Variables such as community size, location, and budget can create challenges for healthcare administrators attempting to utilize EHR, but special initiatives have been enacted to promote and assist its adoption. The Health Information Technology for Economic and Clinical Health (HITECH) Act of 2009 included EHR adoption as a primary objective. Incentive programs such as the Medicare Access and CHIP Reauthorization ACT (MACRA), the Front Community Health Integration (FCHI) project, and financial assistance from the Health Resources and Services Administration (HRSA) test different methods to realize the HITECH goals quickly and efficiently. Their focus is specific to rural healthcare, and trends suggest positive headway is being made.

Rural Healthcare Implementation Progress and Challenges

From 2014 onward, basic EHR adoption has increased in rural areas, and now eight out of 10 rural and critical access hospitals have adopted basic EHR. [1] This trend reflects positive movement toward complete EHR implementation, and it still has to overcome the hurdles associated with rural/small hospitals – lack of resources, budget, and competition. [2] EHR allows hospitals to store and share patient data with such efficiency it directly affects the level of care given. However, its implementation requires training and financial resources unavailable to provincial areas.

MACRA is an example of an incentive program that may be less beneficial than intended. It required certain EHR standards to be met by 2017 that rural healthcare facilities could not attain. The result was inflexible guidelines and lack of an accommodation that hurt the rural organizations who needed assistance. Fortunately, the National Rural Health Association advocated and continues to advocate for accommodation and assistance so unnecessary penalties aren’t levied. [3]

The Frontier Community Health Integration project is more focused on testing new models for healthcare in less populated areas that would meet MACRA requirements. [2] Ten hospitals were selected to take part, and reimbursement was provided to cover interruptions so the hospitals wouldn’t shoulder extra pressure. Additionally, HRSA provided $16 million toward rural EHR improvement initiatives. [2] Given the financial backing, rural hospitals could adapt faster and improve the population health trends.

Basic vs. Certified EHR Trends

Basic EHR are electronic storage and sharing systems for information implemented in at least one unit within the hospital and may not be interoperable. Certified EHR are electronic storage and sharing systems approved by the Department of Health and Human Services that are interoperable. While the ultimate goal of healthcare departments and initiatives is to integrate certified EHR to enable seamless interoperability, implementing any type of EHR is the current campaign. Currently, six out of 10 hospitals have transferred to basic EHR in every state. [1] This is a substantial increase from 2008, and works toward the eventual goal of EHR saturation within healthcare.

Along with an increase in basic EHR adoption, healthcare organizations adoption of certified EHR has remained high. [1] This trend is likely due to the approval and interoperability of certified EHR systems as opposed to basic EHR systems that will likely need to be replaced in the future. Although basic EHR works for the hospital and may be more affordable, the future of healthcare patient records includes interoperability.

Further EHR Trends

Deeper investigation into EHR trends reveals its adoption can be measured on a national level, but varies between specialized hospitals. Basic EHR adoption within children’s hospitals is lower than general hospitals. [1] This trend can be explained due to the type of patients and illnesses a children’s hospital handles. Until recently, pediatric care handled severe illnesses that were resolved during the patient’s stay, so the need to share patient records wasn’t as great. [4] Now, preventative care is identifying chronic behaviors such as behavioral disorder or obesity in pediatrics which needs to be shared with other healthcare organizations [4].

EHR technologies are trending towards full implementation nationwide to benefit patient care and administrative responsibilities. Healthcare administrators take on more duties with the advancement of treatment and illness discovery. Traditional records minimize their effectiveness and efficiency, so initiatives and regulation are promoting an immediate implementation of EHR. Rosalind Franklin

In the world of healthcare innovation, few discoveries compare to the discovery of the structure of deoxyribonucleic acid (DNA). Although James Watson, Francis Crick, and Maurice Wilkins were awarded the Nobel Prize for discovering the famed double helix in 1962, controversy followed.

There’s no doubt that Rosalind Franklin’s pioneering work with x-ray photography revealed DNA in such a way that it’s structure could be identified. However, she fell just short of making the breakthrough discovery herself, and it is rumored that a colleague showed her work to Watson and Franklin, who then published their findings, essentially beating her to the punch. Rosalyn Sussman Yalow

Interested in math and science from a young age, Rosalyn Yalow studied physics in college during the 1930s and segued her prowess with the topic into collegiate teaching engagements. She later became interested in research on the use of radioactive isotopes for medical applications and developed Radioisotope Services with grant money from the U.S. Department of Veterans Affairs.

Along with Dr. Solomon Berson, Yalow developed radioimmunoassay (RIA), a screening technique used to discover previously imperceptible substances like drugs or hormones in the body. This technique has most notably been used in blood banks to determine whether donations are suitable for use, and the process won Yalow the Nobel Prize in 1977. [2] [4]

Francoise Barre’-Sinoussi

This French-born scientist began her illustrious career as a volunteer at the prestigious Institut Pasteur in Paris, where she joined a team studying the relationship between retroviruses and cancers in mice. After completing a PhD in 1974, Barre’-Sinoussi participated in several high-profile research projects before she was invited to study a new disease in 1982 to determine whether in might be caused by a retrovirus.

Working with Luc Montaigner and others, she discovered the retrovirus known as Human Immunodeficiency Virus (HIV) in 1983. After that she collaborated on HIV and AIDS research and educational efforts in resource-limited countries in Africa and Asia, and she was appointed head of the Biology of Retroviruses Unit at Institut Pasteur in 1992. She received the Nobel Prize for her work in 2008. [5]

Antonia C. Novello

A native of Puerto Rico, Novello moved to the U.S. to complete her internship and fellowship before opening a pediatric private practice in Virginia in 1976. She eventually moved into public health administration and was appointed Surgeon General in 1990 by the Bush administration.

She was not only the first woman to hold this position, but also the first person of Hispanic heritage to do so, and she focused on combating health issues among minorities, women and children, most notably waging a campaign against big tobacco and the use of figures like Joe Camel that blatantly appealed to the youth market. Regarding honesty; it’s fairly easy to identify who is honest by seeing and evaluating their actions. Recognizing good character and honor, however, may prove more difficult, because these traits can be demonstrated (or not demonstrated) in various ways.

While it may initially seem impossible to define high ethical standards, it’s really not. The American Association of Professional Coders (AAPC) publishes standards that outline workplace conduct, presentation, work ethic and communication.
